Mind Connections is an Ontario based multi-disciplinary rehabilitation company, providing services to severely injured individuals including ones with traumatic brain injuries, spinal cord injuries, severe orthopedic injuries and mental health issues.
Our team comprises of neuro-surgeons, orthopedic surgeons, neurologists, psychiatrists, physiatrists, psychologists, occupational therapists, social workers, speech language pathologists, dietitians, rehabilitation support workers and personal support workers working collaboratively with the clients. Social workers are responsible for completing assessments and treatment for our clients. They assist us in functional rehabilitation to the clients in their environment to maximize the functional performance of our clients.

We are currently seeking a motivated and enthusiastic social worker to join our team.
Qualifications:
- Masters degree in Social worker
- Strong writing and communication skills
- Critical thinking and decision making skills
- Effective time management and organizational skills
- Experience working under the Statutory Accident Benefits Schedule (SABS) will be considered an asset.
Responsibilities will include:
- Providing assessments based on presenting impairment, disability, or handicap, using standardized assessment tools
- Formulating treatment plans using evidence-based information, relevant best practice guidelines and clinical expertise
- Actively engaging clients, families, and caregivers in treatment programs to address client centered goals
- Linking clients/caregivers with appropriate community partners/services/agencies
- Evaluating and documenting the effectiveness of the intervention, client’s response, and progress towards goal attainment
- Completing clinical records according to college standards
- Communicating effectively with clients, families, referral sources including team members and office staff.
